COCCYGODYNIA ( THE COCCYX PAIN )

The pain of coccyx is called coccygodynia. It is said to be a chronic painful condition that occurs in this region as a result of some trauma in the tail spine area. Traumatic conditions, such as falling on a buttock, crashing into a tail's head, sitting for a long time, cause pain in the tail's throat. It is more common in women due to anatomical structures. Especially the prevalence during pregnancy and post-pregnancy period increases.

Symptoms are pain in the locus area of the tailbone, particularly in sitting, and swelling and pain in the anal area. Careful examination and x-ray filming is necessary and sufficient for diagnosis. Evaluation may also be necessary in terms of other possible anomalies. It is important to distinguish between infectious conditions such as hair follicles. In some patients with X-ray films, the spine of the tail bone is visible. However, at least half of the patients do not show any abnormality in X-ray films.

 Treatment of;

 1- Pain Management Methods (nerve blockage)

 The most effective treatment for spinal cord pain is the nerve blockage method for the nerves of that region.

 This method called 'Ganglion Impar Blockage' is performed by displaying the location of the needle in the presence of a special x-ray machine. It provides long-term pain relief. Mixture of cortisone and local anesthetic drug can be given as medicine. Ultrasonic control before injection is useful. Injection can also be done in the presence of ultrasound imaging. But ultrasound alone is not enough because it does not show the bone well. In our clinic, we first check with ultrasound to see if there is an obstruction (cyst, infection, abscess at the needle entrance site ...). After that, we go under the bone in the presence of the fluoroscopy device and apply medication and radiofrequency to the neural network.

Radiofrequency (nerve burning) and cryo-neurolysis (nerve freezing) treatments have been successfully applied in recent years.

 2- Superficial injections

Cortisone and anesthetic drug injections made without using any imaging device have a low success rate and the duration of action is rather short. Because it is not possible to overcome the bone in blind injections.

 3- Laser and other physical therapy methods

 Limited effects are available.

 4- Manual therapy

 The effect of manual therapy in the treatment of cauda equina pain is not clear. This issue is still controversial and it is well known that the complaints are effective in the patients who have just begun, while they are ineffective in delayed cases.

 5- Protection

 Using a sitting cushion, sitting on a soft seat, hot water treatment can be applied to such methods.

 6- Surgery

 If the methods we mentioned are unsuccessful, we are trying to be treated with surgery. The extraction of the caudal bone can be of benefit in treatment.
